Cook pasta in a large pot of salted boiling water until just shy of al dente. Reserve 1/2 cup pasta water, then drain.
While pasta cooks, heat oil or butter in a skillet over medium heat. Add sliced onion with a pinch of salt and cook 6–8 minutes until soft and golden.
Add garlic for 30 seconds if using, then stir in chopped tomato. Cook 2–3 minutes until tomato softens; add a pinch of sugar if it’s too tart.
Pour in cream (or milk + butter), lower heat, and simmer gently 2–4 minutes until sauce slightly thickens. If too thick, loosen with reserved pasta water a tablespoon at a time.
Stir in drained pasta and grated cheese, tossing to coat. Season with salt, lots of black pepper, and red pepper flakes if you like heat.
Finish with fresh herbs or a tiny squeeze of lemon to brighten. Serve immediately and enjoy — don’t be shy with the cheese.
